Job Details

Key Responsibilities:

Perform comprehensive patient assessments and develop individualized care plans
Administer medications, treatments, and IV therapies as prescribed
Monitor residents' conditions and report changes to physicians and care teams
Maintain accurate and timely clinical documentation in accordance with state and federal regulations
Ensure compliance with infection control, safety, and quality standards
Communicate effectively with residents, families, and interdisciplinary team members
Provide patient and family education on health conditions and care needs
