This role is primarily remote, facilitating flexible working arrangements geared towards accommodating the unique needs of our senior clients. While most interactions and assessments will be conducted virtually, you may occasionally need to visit clients in their homes or relevant community locations, ensuring continued engagement and personalized care. • Conduct comprehensive psychosocial assessments for seniors, delving into emotional, mental, and social dimensions to inform care planning.

• Offer supportive counseling to clients, facilitating conversations on emotional transitions, life challenges, and changes in family dynamics.

• Advocate tirelessly for client rights, providing access to critical community resources such as support groups, healthcare services, and financial assistance.

• Facilitate open communication between family members to resolve conflicts that may arise in the course of caregiving and support.

• Maintain meticulous documentation of client interactions, assessments, care plans, and case histories to ensure compliance and enhance service delivery.

• Regularly monitor clients’ well-being through virtual check-ins and data management, including follow-up assessments and progress tracking.

• Stay updated on current best practices in social work and community services, ensuring adherence to ethical standards and policies.

Your primary focus will be on seniors, particularly those dealing with the complexities of aging, health transitions, and substance use recovery. • Master's degree in Social Work from an accredited institution is mandatory.

• Valid and current state licensure as a Master Social Worker (LMSW) is required.
