Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients and families to pinpoint emotional, social, and environmental challenges related to their current health status.

• Formulate, develop, and implement individualized care plans that leverage therapeutic interventions designed to improve patient outcomes and quality of life.

• Collaborate with the healthcare team to involve patients and families in developing and executing their care plans, ensuring that their voices are prioritized and respected.

• Facilitate discharge planning that navigates through a multidisciplinary continuum of care, working with consulting physicians and third-party payers to create seamless transitions for patients.

• Provide education and counseling to patients and families regarding strategies for adapting to significant health-related changes and challenges.

• Utilize crisis intervention techniques as necessary, and offer brief or long-term individual, group, and family therapy, tailoring your approach to the specific needs of those you serve.

• Act as a valuable resource for other staff members, providing support, guidance, and expertise in psychosocial aspects of patient care.
\n
• Evaluate oncology patients for symptoms of depression and anxiety, providing timely counseling to support their mental health needs.
\n
• Take part in an on-call rotation to address urgent after-hours situations, ensuring that support is available when it is most needed.
\n
• Facilitate involuntary examinations under the Baker Act when needed, ensuring the safety and care of patients requiring immediate attention.
\n
• Present at educational conferences to share insights as an expert in psychosocial oncology practices, contributing to the professional development of your peers.

• Engage with the local community through education, consultation, and volunteering efforts to enhance available services and resources for cancer survivors.

Patient Population / Client Focus\n

This position primarily serves patients in hospice care, many of whom face life-limiting illnesses. Family dynamics, bereavement issues, and diagnosis-related stress are prevalent in this setting. The LCSW will engage with a diverse population, addressing varied cultural backgrounds, religious beliefs, and social factors that influence patients’ experiences and coping mechanisms.

Qualifications\n

• Must hold a valid and active license as a Clinical Social Worker (LCSW) in the State of Texas.

• Bachelor's degree in Social Work; advanced education such as a Master’s in Social Work (MSW) preferred.

• Strong understanding of hospice and palliative care frameworks and familiarity with social work principles and practices applicable to serious illness.

• Exceptional communication and interpersonal skills, with the ability to connect empathetically with patients and their families.

Compensation and Benefits\n
