This full-time position operates in a remote capacity, offering flexibility while ensuring that our clients receive uninterrupted care. Typically, work hours are scheduled from Monday to Friday, 8:00 AM to 4:30 PM, though occasional evening or weekend hours may be necessary based on client needs. You will have the necessary tools and resources, including access to advanced electronic health record systems, to effectively manage your caseload and enhance your interactions with clients and their families.

Key Responsibilities\n

As a Licensed Master Social Worker at Clarity Pathways Counseling, you will:

• Conduct comprehensive assessments to evaluate seniors' mental, emotional, and social health, identifying specific needs for interventions.

• Collaborate with clients and their families to create personalized care plans that reflect their individual goals, preferences, and cultural backgrounds.

• Provide counseling services aimed at supporting seniors through transitions such as health challenges or changes in living situations.

• Advocate for clients to ensure they have access to necessary community resources, programs, and support services.

• Work in collaboration with an interdisciplinary team, coordinating care initiatives alongside healthcare professionals and caregivers.

• Regularly check in with clients to monitor progress and address any emerging concerns or safety issues in their living environments.

• Facilitate communication among family members to help resolve conflicts regarding the care of senior clients.

• Educate clients and families about available community resources and programs that encourage social engagement and enhance well-being.

• Maintain detailed and accurate documentation of client interactions, care plans, progress notes, and any other relevant information to ensure continuity of care.

Qualifications\n

Compensation and Benefits\n
