This position is primarily remote, allowing you to tailor your working hours while providing essential services to seniors across various locations. Key Responsibilities\n

• Conduct in-depth psychosocial assessments to evaluate the emotional, mental, and social needs of senior clients.

• Collaborate closely with clients and their families to create individualized care plans tailored to their goals, preferences, and circumstances.

• Provide counseling services and emotional support, helping clients navigate significant life transitions such as health changes, loss, or bereavement.

• Act as an advocate for client rights, ensuring that seniors have access to necessary community resources and that their voices are heard.

• Work hand-in-hand with a multidisciplinary team—including nurses, caregivers, and other healthcare professionals—to coordinate and enhance holistic care.

• Regularly monitor client well-being through scheduled visits, assessing changing conditions and advocating for any necessary adjustments to care plans.

• Facilitate discussions between family members to resolve conflicts related to senior care and ensure cohesive support.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
