Clarity Pathways Counseling is seeking a highly skilled and compassionate Licensed Master Social Worker (LMSW) to join our dedicated remote team. In this full-time role, you will provide essential social work services to seniors within a school environment, focusing on enhancing their overall well-being and quality of life.

Work Environment\n

This position is fully remote, allowing for flexibility while working from home to support our senior clients. Although primarily virtual, there may be occasional in-person engagements, particularly for assessments or meetings that require direct interaction. The work hours are set from Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of additional hours during evenings or weekends, contingent on client needs.

Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, taking into account their emotional, mental, and social factors to create a comprehensive understanding of their situation.

• Collaborate with clients and their families to develop personalized care plans that reflect their specific goals, preferences, and the challenges they encounter.
\n
• Offer counseling and emotional support, assisting seniors and their families in navigating transitions, such as relocation to assisted living or dealing with the loss of loved ones.
\n
• Advocate for clients' rights, guiding them through access to community resources and ensuring they receive appropriate support and services.
\n
• Work closely with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate and enhance the care provided to seniors.
\n
• Regularly visit clients (virtually or in person) to monitor their well-being, assess living conditions, and proactively address any concerns or safety issues.
\n
• Facilitate family communication, helping to resolve conflicts and fostering a supportive environment for the client’s care.
\n
• Educate clients and families on available community resources and support groups that can enrich their social engagement and well-being.
\n
• Maintain accurate documentation, including client assessments, care plans, and progress notes, ensuring adherence to ethical standards and compliance with regulations.

• Stay updated on current social work practices, policies, and best practices to offer the highest standard of care.

Your primary focus will be seniors, who often face unique psychosocial challenges including loneliness, mental health concerns, and age-related transitions. Your role is pivotal in recognizing these challenges and facilitating access to both emotional and tangible support, thus positively impacting their lives and the lives of their families.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Master Social Worker (LMSW).
\n
• A minimum of two years of experience in a social work role, with a preference for experience in a community center or geriatric setting.

• Solid understanding of social work principles and aging-related issues.

• Strong interpersonal skills that foster trust and empathy with clients and families.

• Proficiency in electronic health record (EHR) systems and related software for documentation.

Compensation and Benefits\n
