Key Responsibilities\n

• Conduct thorough psychosocial assessments of clients, taking into account emotional, mental, and social well-being.

• Develop and implement personalized care plans alongside seniors and their families, addressing specific goals and preferences.

• Provide counseling and emotional support to navigate various life transitions such as health changes, relocation, or loss.

• Advocate for clients, ensuring they access necessary resources and services, including healthcare and community support.

• Work closely with interdisciplinary teams—including teachers, nurses, and caregivers—to optimize care strategies for seniors.

• Regularly monitor clients' conditions through scheduled visits, assessing safety and well-being while updating care plans accordingly.

• Facilitate communication and conflict resolution among family members regarding care decisions to promote a harmonious environment.

• Educate families on available local resources, support networks, and programs aimed at enhancing seniors' social engagement.

• Maintain accurate records of client interactions, documenting assessments, care plans, and progress notes through our EHR.

• Stay informed on current best practices and maintain compliance with relevant regulations and ethical guidelines.

Patient Population / Client Focus\n

This position focuses specifically on senior adults, many of whom face unique life challenges related to aging. You will work with clients who may be experiencing health-related transitions, social isolation, and mental health concerns. Your expertise will be critical in fostering a supportive atmosphere that empowers clients to overcome hurdles and engage fully in life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LMSW licensure in the state of Texas.

• A minimum of two years' experience in social work, preferably within a community or geriatric setting.

• Demonstrated understanding of aging-related issues and the available resources relevant to adults in the community.

• Strong assessment, counseling, and problem-solving skills with a compassionate approach.
