Work Environment\n

This role provides a unique work environment as it allows flexibility through remote work arrangements. Our hospice center team is dedicated to delivering high-quality care in a supportive and collaborative atmosphere. You will engage with clients from the comfort of their homes, performing remote assessments and planning care strategies to improve their well-being. Occasional evening or weekend hours may be required to accommodate client needs and ensure continuous support. The work schedule from Monday to Friday is designed to be flexible to meet the demands of our clients and the organization.

Key Responsibilities\n

As a Licensed Master Social Worker, your duties will include:

• Conducting thorough assessments of clients' psychosocial needs, considering emotional, mental, and social factors that influence their quality of life.

• Collaborating closely with clients and their families to create personalized care plans tailored to their specific goals and preferences.

• Providing empathetic counseling and emotional support to clients and their families, assisting them in navigating the complexities of end-of-life care and other life transitions.

• Advocating for clients' rights and ensuring access to essential community resources and services that enhance their quality of life.

• Working alongside an interdisciplinary team of healthcare professionals to streamline care coordination and improve patient outcomes.

• Regularly visiting clients to monitor their well-being, assess living conditions, and address any emerging concerns or safety issues.

• Facilitating communication between family members, helping to resolve conflicts and ensuring everyone is informed and involved in the care process.

• Educating clients and families about community resources, support groups, and programs that foster social engagement and emotional resilience.

• Maintaining accurate and confidential documentation of client interactions, assessments, and progress notes in compliance with ethical standards.

• Staying current on best practices in social work, policies, and regulations relevant to hospice care and mental health.

Patient Population / Client Focus\n

This role will predominantly focus on senior clients receiving hospice care, who often face complex emotional and mental health challenges related to aging, chronic illness, and end-of-life issues. Your expertise will help these individuals and their families confront difficult decisions and transitions while ensuring they feel supported and understood.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• A current and valid state license as a Licensed Master Social Worker (LMSW).

• A minimum of two years of professional experience in a social work role with a preference for experience in hospice care or geriatric settings.

• In-depth knowledge of social work principles, particularly surrounding aging and available community resources.

• Strong interpersonal skills with a demonstrated ability to build rapport and trust with clients and their families.

• Exceptional communication skills for effective collaboration with healthcare professionals and clients.

While not required, experience in hospice, palliative care, or geriatric social work is highly valued. Familiarity with community health resources and local agencies that support senior care will be considered a plus.

Compensation and Benefits\n

The compensation for the Licensed Master Social Worker position ranges from $60,000 to $100,000 per year, depending on experience and qualifications. Benefits include:
