Work Environment\n

This position is primarily remote, allowing you to connect with clients and their families in their homes while maintaining flexibility in your schedule. Despite the remote nature of the work, you will have the opportunity for regular face-to-face interactions during home visits. Our core working hours are Monday through Friday, from 8:00 AM to 4:30 PM, with occasional flexibility for evening or weekend visits based on the specific needs of clients and their families. On-call support may also be required for urgent situations. You will receive ongoing support from a passionate team of healthcare professionals who value your contributions and expertise.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify emotional, mental, and social needs of elderly clients, ensuring a holistic approach to their care.

• Develop and implement individualized care plans in collaboration with clients and their families, addressing their specific preferences, goals, and necessary resources.

• Engage in compassionate counseling sessions with clients and their families, aiding them through life transitions including grief, loss, and changes in health status.

• Act as an advocate for clients, ensuring their rights are upheld and assisting with access to community resources, healthcare services, and support groups.

• Regularly perform home visits to monitor client well-being, assess living conditions, and proactively address concerns related to safety and quality of life.

• Facilitate healthy communication among family members and assist in conflict resolution to promote transparency and collaborative decision-making in care.

• Maintain accurate and detailed documentation within electronic health record systems (EHR) to ensure compliance with ethical standards and regulatory requirements.

• Stay current on social work best practices, policies, and developments in hospice care to ensure the highest level of service and adherence to ethical practices.

Your work will primarily involve elderly clients in hospice care who are facing terminal illnesses. You will also interact with their families, who require guidance and support through emotionally challenging times. Your role will be pivotal in helping to promote quality of life even in the final stages, advocating for dignity and respect at all times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of Colorado.

Compensation and Benefits\n
