Key Responsibilities\n

• Conduct comprehensive assessments to understand and address the unique needs of each patient.

• Maintain effective communication with supervisors, direct care staff, family members, and guardians regarding patient progress and treatment plans.

• Develop and provide a tentative schedule to supervisors outlining appointments, home visits, and planned observations to monitor patient progress.
\n
• Coordinate home visits to conduct in-home observations, perform chart reviews, engage in staff interviews, and analyze data for informed decision-making.
\n
• Ensure that the implementation of the treatment program plan is effective and adjusts as necessary based on patient needs and feedback.
\n
• Actively participate in Interdisciplinary Team Meetings and contribute valuable insights to enhance patient care.
\n
• Document monthly and quarterly progress notes to track patient development and treatment efficacy.
\n
• Organize training sessions to ensure that all staff members are proficient in implementing the program plan and maintaining accurate documentation.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to address the continued needs of individual clients.
\n
• Maintain meticulous records to ensure that vital information is accessible to relevant parties and complies with legal requirements.

• Serve as a liaison with the public and the mental health community, aiding in the development of programs to advance community health initiatives.

• Assist in creating and refining procedures for training materials aimed at optimizing client programs and support services.

The LPC will work with a diverse client population, including children, adolescents, and adults facing a variety of challenges such as anxiety, depression, trauma, and relationship issues. Qualifications\n
