Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their mental health needs and develop personalized counseling plans.

• Facilitate effective communication with supervisors, direct care staff, families, and relevant parties to monitor and report on clients' progress.

• Design a structured schedule that outlines appointments, home visits, and necessary observations to track patient advancement.

• Coordinate and perform home visits, ensuring compliance with program requirements through observations, chart reviews, and data collection.

• Oversee the implementation and effectiveness of personalized care plans to ensure positive patient outcomes.

• Engage actively in Interdisciplinary Team Meetings to share insights and best practices regarding patient care.
\n
• Prepare detailed monthly and quarterly progress notes that document patients' developments and therapeutic advancements.

• Organize and conduct training sessions for staff to ensure compliance and effectiveness in implementing program plans and documentation standards.

• Provide ongoing support through follow-up interviews and observations to gauge continuous client needs.
\n
• Maintain accurate and accessible records to ensure information privacy and support easy access for relevant stakeholders.

• Act as a liaison with the broader mental health community to enhance program development initiatives.

• Assist in the creation and refinement of training materials designed to enhance therapeutic programs.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted LCPC license in the state of Maryland is mandatory.
