Key Responsibilities\n

• Manage a specific caseload of patients, assessing their unique social, emotional, and financial situations to inform their overall treatment plans. Utilize a systemic approach to address psychosocial barriers that hinder timely transitions through the healthcare continuum.

• Lead and coordinate efforts within the healthcare team to navigate protective service issues, advance directives, and life care planning. Provide crisis intervention and ongoing support to families and patients facing complex psychosocial challenges.

• Collaborate with case managers and the healthcare team to deliver education, resources, and referrals. Ensure that psychosocial and socioeconomic factors are thoroughly integrated into the treatment and discharge plans.

• Engage in supervision and mentorship of Social Work Interns and less experienced staff, driving standard practices that enhance service delivery and patient care outcomes.

• Oversee and implement resources for patients to minimize the risk of readmission, ensuring a seamless integration of care that utilizes public, private, and philanthropic funding effectively.

• Act promptly in addressing psychosocial service needs to prevent unnecessary delays in patient treatment and discharge. Manage multifaceted biopsychosocial cases with an aim to expedite care transitions.

• Participate in weekly rounds, patient care conferences, and other collaborative meetings to discuss cases and refine care coordination efforts.

• Remain on-call as required by department leadership, ready to support the team in crisis situations that arise, thereby ensuring continuity of care.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
