Remote Licensed Clinical Social Worker for Hospice Care

The Life Essence Center is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our dedicated team in a remote capacity. Key Responsibilities

• Conduct thorough mental health assessments to evaluate the psychological needs of patients, taking into account personal histories, current concerns, and any risk factors.
• Develop and implement personalized treatment plans tailored to the individual needs of patients,focused on promoting their mental health and emotional stability in the hospice setting.
• Utilize evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and trauma-informed approaches, to manage a range of emotional and psychological challenges faced by patients and their families.
• Facilitate both individual and family therapy sessions that include a focus on coping strategies for end-of-life issues, grief, and loss.
• Collaborate with other healthcare professionals, such as chaplains and nurse case managers, to coordinate care and ensure comprehensive support across all aspects of patient health.
• Provide crisis intervention and support during emotional emergencies, employing de-escalation techniques and making necessary referrals for advanced care.
• Educate patients and their families about mental health, coping mechanisms, and available resources, fostering a better understanding of the psychological aspects of hospice care.
• Maintain precise and confidential documentation of assessments, treatment plans, and progress notes in compliance with regulatory and ethical standards.
• Actively participate in team meetings and training sessions to remain current on best practices in mental health care specific to the hospice environment.

Patient Population / Client Focus

This role primarily involves working with patients who are receiving end-of-life care, along with their families. You will provide emotional support in dealing with serious illnesses, grieving, and planning for end-of-life decisions. Your expertise will enhance the quality of life for individuals adjusting to complex emotional challenges during this critical time.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A current and valid LCSW licensure in the state of New Jersey.
• A minimum of two years of post-licensure experience in mental health, preferably in hospice or palliative care settings.
• In-depth knowledge and understanding of mental health disorders, evidence-based treatment options, and trauma-informed care principles.
• Strong skills in conducting mental health assessments and formulating effective treatment plans.
• Proficiency in providing individual and family therapy using diverse therapeutic methodologies.
• Exceptional crisis intervention skills, along with the ability to maintain composure in high-stress situations.
• Excellent interpersonal and communication skills to build trust and rapport with patients and families from varied backgrounds.
• Proficient in using electronic health record (EHR) systems to document clinical interactions effectively.

Preferred Experience

• Experience working within a hospice, palliative care, or end-of-life care setting.
• Familiarity with community resources and services available for patients and families facing terminal illnesses.

Compensation and Benefits

The salary range for this position is between USD 80,000 and 100,000 annually, commensurate with experience and qualifications.
