Work Environment\n

The position is remote, allowing for flexibility and work-life balance while still being an integral part of our hospice center’s care team based in San Jose, California. Although primarily a work-from-home role, occasional visits to the hospice facility may be necessary for patient assessments or collaboration with other healthcare professionals. The job requires some evening and weekend hours to meet client needs effectively, thereby providing responsive and tailored support to those we serve.

Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough psychosocial assessments and developing personalized treatment plans that address the unique needs of each patient.

• Implementing evidence-based interventions such as solution-focused therapy, motivational interviewing, and behavioral activation to facilitate positive treatment outcomes.

• Monitoring and documenting patient progress closely, adjusting strategies as needed to ensure that therapeutic goals are met.

• Collaborating with a multidisciplinary team, which includes physicians, psychologists, and other healthcare providers, to create a holistic care approach.

Qualifications\n

To be considered for this role, you must possess:

Compensation and Benefits\n
