Key Responsibilities\n

• Conduct thorough patient intakes, emphasizing diagnostic and functional evaluations to inform treatment plans.

• Provide focused therapeutic interventions for individuals in need of mental health support, addressing both immediate and long-term needs.

• Collaborate with primary care providers to identify patients who may benefit from specialty mental health services.

• Initiate and maintain follow-up communication with patients, assessing progress and adapting treatment approaches as necessary.

• Develop relapse prevention plans and facilitate stability in patients at risk for deterioration of mental health.

• Document patient progress meticulously in treatment charts, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.

• Assist patients in adhering to prescribed medical treatments, providing strategies to manage potential medication side effects.
\n
• Engage in clinical consultations among team members to enhance the effectiveness of behavioral services and evaluate program outcomes.

• Provide educational resources and self-management skills to empower patients in their recovery journeys.

• Maintain a thorough knowledge of community resources to support and advocate for patients’ needs.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• A current Licensed Professional Counselor (LPC) license.

Preferred Experience\n

Compensation and Benefits\n

This full-time position offers a competitive salary in the range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
