Key Responsibilities\n

As a Clinical Social Worker at Healing Springs Family & Wellness Center, your core responsibilities will include:

• Conducting comprehensive psychosocial assessments that help identify emotional, social, and financial needs of patients and their families.

• Collaborating with medical teams to devise and implement holistic care plans that address both physical and psychological aspects of patient well-being.

• Providing individual and group counseling sessions to patients, focusing on coping strategies for adjustment, grief, and emotional distress.

• Coordinating with external community organizations to arrange essential post-discharge services, ensuring continuity of care for patients after leaving our facility.

• Offering emotional support and education to patients and families about treatment options and available resources throughout the healthcare journey.

• Actively participating in hospital committees and contributing your expertise on social work matters to enhance organizational policies and procedures.

• Delivering crisis intervention and support to patients and families, always prioritizing their immediate emotional needs.

• Developing and executing transition plans upon discharge, connecting patients with appropriate community resources.

• Maintaining accurate and timely documentation as required by hospital policies and legal regulations to ensure high standards of care.

Patient Population / Client Focus\n

In this role, you will be primarily focusing on providing support to patients within the hospice context. Our patients often face complex emotional and psychological challenges as they navigate the end-of-life process. You will work closely with families, ensuring they have access to the resources they need while providing crucial education on pain management and end-of-life options.

Qualifications\n

To succeed in this position, candidates must possess the following qualifications:
