This is a full-time remote position based in Sunnyvale, California, providing flexibility and the ability to work from home. Key Responsibilities\n

As the Licensed Clinical Social Worker, you will:

• Conduct thorough psychosocial assessments for patients and their families, identifying underlying emotional and social needs.

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare team members.

• Facilitate both individual and group psychotherapy sessions, delivering evidence-based therapeutic interventions.

• Coordinate with community agencies and resources to enhance support systems surrounding our patients.

• Document treatment plans and patient progress in compliance with legal and administrative standards, ensuring all records are accurate and timely.

• Advocate for patient needs across disciplinary teams, helping to navigate complex healthcare systems.

• Respond effectively to crisis situations, utilizing clinical judgment and calm communication.

Patient Population / Client Focus\n

Your primary focus will include patients accessing hospice services, who often face complex emotional challenges associated with terminal illnesses. You will also support their families, providing grief counseling and assisting with anticipatory mourning processes, thereby fostering resilience and coping strategies.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

• Valid California License as a Licensed Clinical Social Worker issued by the California Board of Behavioral Science Examiners.

• A minimum of 2 years direct experience in providing mental health services, preferably in health-related or hospice settings.

• Evidence of commitment to ongoing professional development through continuing education.

Preferred Experience\n

• Experience with Department of Mental Health billing procedures and documentation processes.

• Background in parent-child intervention and knowledge of child development, parenting dynamics, and child abuse/neglect issues is a plus.
