Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients' mental health needs, strengths, and available resources within the hospice context.

• Provide diverse therapeutic interventions, including individual, family, and group therapy, drawing upon evidence-based modalities tailored to the unique challenges of hospice care.

• Utilize therapeutic approaches such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care to support clients and their families during critical times.

• Develop and implement individualized treatment plans that encompass measurable goals and interventions, regularly reviewing progress with each client.

• Collaborate closely with a treatment team, including psychiatrists and psychologists, to coordinate comprehensive care that addresses both mental and physical health.

• Facilitate group therapy and support sessions via virtual platforms to foster community and shared experiences among clients and their families.

• Provide crisis intervention and safety planning, responding to urgent client needs and ensuring emotional support is available when necessary.

• Maintain accurate documentation aligned with ethical and legal standards, ensuring compliance while facilitating open communication with the care team.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
