Key Responsibilities\n

• Conduct thorough initial assessments and ongoing evaluations of clients to tailor individualized care plans that address mental health needs specific to hospice care.

• Apply advanced behavior management techniques to support clients coping with severe illness, bereavement, and end-of-life issues.

• Provide individual and family counseling sessions, adapting therapeutic strategies to meet diverse client needs.

• Work collaboratively with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement holistic treatment plans.

• Empower families by sharing educational resources and coping strategies to facilitate developmental success and emotional resilience during critical transitions.

• Implement evidence-based therapeutic practices, documenting progress and adjusting treatment approaches as necessary to achieve positive outcomes.

• Maintain accurate and timely clinical documentation in accordance with state regulations and organizational policies, using electronic health records (EHR) systems.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active Licensed Clinical Social Worker (LCSW) credential in California.

Compensation and Benefits\n
