Key Responsibilities\n

• Conduct comprehensive psychosocial assessments by engaging with patients and their families through interviews and observations, as well as reviewing medical records to identify emotional, social, and environmental factors affecting recovery and quality of life.

• Offer empathetic counseling and emotional support to patients and their families, particularly addressing adjustment issues related to serious illness and end-of-life concerns.

• Collaborate closely with our interdisciplinary rehabilitation team to create personalized treatment plans focused on the psychosocial needs of each patient, ensuring all aspects of their well-being are considered.

• Assist patients in navigating and accessing community resources, including support groups, vocational rehabilitation programs, and housing assistance, to enhance their overall support network during challenging times.

• Keep detailed documentation of patient interactions and progress in alignment with organizational standards, ensuring effective communication with the entire care team.

• Stay updated on the latest social work practices and policies to continually provide the best possible care.

Patient Population / Client Focus\n

You will work predominantly with patients in a hospice setting, often facing life-limiting illnesses and their families. This role requires a deep understanding of grief, loss, and coping mechanisms as you support patients and their loved ones through a transformative and emotional journey. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n
