Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments to identify mental health, substance use, and medical needs in patients, ensuring all services provided are culturally appropriate for diverse populations.

• Facilitate counseling sessions for individuals, groups, and families, focusing on intake, assessment, treatment planning, and crisis intervention, while proactively addressing the unique challenges of end-of-life care.

• Serve as a liaison between patients and community resources, connecting families to essential services such as medication assistance programs and local support groups.

• Document clinical assessments, progress notes, and treatment plans accurately and timely in accordance with state regulations and organizational standards, utilizing electronic health record systems.

• Collaborate with interdisciplinary teams, including healthcare providers and quality improvement personnel, to enhance service delivery and ensure best practices are upheld.

Patient Population / Client Focus\n

Your primary focus will be on adult patients in hospice care, as well as their families, who are navigating serious illness and end-of-life decisions. You will be instrumental in providing emotional support, education about the hospice process, and facilitating discussions around advanced care planning. Understanding each client’s unique circumstances is crucial, as is the ability to empathize with grief and loss while providing vital counseling resources.

Qualifications\n

• Must possess and maintain an active Licensed Clinical Social Worker (LCSW) license in the state of California.

• A Master’s degree in Social Work (MSW) is required, with a minimum of three years of post-licensure clinical experience in a relevant setting.
