Work Environment\n

This role will be primarily remote, allowing for flexibility in your schedule while ensuring access to our clients when needed most. Although you will primarily work from home, you will be required to conduct regular visits to the hospice center and clients’ homes as part of your job responsibilities. You will have the opportunity to collaborate with an interdisciplinary team, including nurses, caregivers, and healthcare professionals, to create a supportive framework for our clients.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, considering emotional, mental, and social issues that may affect their well-being.

• Develop personalized care plans in collaboration with clients and their families, focusing on specific goals and preferences that align with their life situations.

• Provide counseling and emotional support to seniors and their families, assisting them in navigating transitions such as health changes and loss.

• Advocate for clients' rights and facilitate access to essential community resources, including healthcare services and support groups.

• Work closely with interdisciplinary teams to coordinate services and improve care quality, including regular communication with healthcare providers.

• Engage in routine visits to monitor clients' welfare, assess living conditions, and address any emerging safety concerns.

• Educate clients and their families about available community resources, programs, and social engagements, fostering a connection to the community.

• Maintain detailed documentation of client interactions, assessments, care plans, and progress notes within electronic health record (EHR) systems.

• Stay updated on current best practices in social work and hospice care, ensuring adherence to ethical standards and organizational policies.

Qualifications\n

Compensation and Benefits\n
