Key Responsibilities\n

• Provide comprehensive individual, group, and family therapy tailored to the specific emotional and psychosocial needs of hospice patients.

• Conduct thorough psychosocial assessments to identify patient challenges and develop customized treatment plans utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Implement trauma-informed care practices to address and mitigate the impact of trauma on patient wellbeing.

• Deliver crisis intervention and case management services within a multidisciplinary team framework, coordinating care with physicians, nurses, and other healthcare professionals involved in patient care.

• Maintain up-to-date, accurate, and confidential documentation of clinical encounters in our electronic health record (EHR) system, ensuring compliance with HIPAA regulations.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care, providing emotional, social, and psychological support during their end-of-life journey. Qualifications\n

• Active and unrestricted Licensed Clinical Social Worker (LCSW) license in California.

Compensation and Benefits\n

The salary for this position ranges from $100,000 to $130,000 annually, depending on experience and qualifications.
