Key Responsibilities\n

• Engage with clients during scheduled appointments, ensuring timely access to therapy as mandated while upholding a patient-centered approach.

• Conduct comprehensive psychosocial assessments and develop customized care plans to address each client’s unique needs, facilitating peaceful transitions for individuals and their families.

• Collaborate with other members of the clinical team to coordinate care, share insights, and employ best practices according to California’s legal and ethical standards.

• Document clinical encounters and progress notes within a 72-hour timeframe post-session, routing them through our designated electronic health record (EHR) system to adhere to professional norms.

• Utilize therapeutic models and evidence-based practices to support clients experiencing grief, loss, and other emotional challenges associated with end-of-life care.

• Maintain a professional demeanor by fostering respectful relationships with clients, families, and colleagues, contributing to a positive workplace and an empathetic patient experience.

Patient Population / Client Focus\n

This role focuses on providing exceptional care to patients enrolled in hospice services, often dealing with critical and complex end-of-life issues. As an LCSW, you will work closely with families to provide support and strategies to handle grief, manage psychological distress, and cope with loss. The LCSW will be integral in facilitating discussions about care preferences and advanced directives, empowering clients to express their wishes during a profoundly sensitive time.

Qualifications\n

• Master's Degree in Social Work from an accredited institution.

Compensation and Benefits\n

This full-time remote position offers a competitive annual salary in the range of USD 120,000 to 130,000, dependent on experience and qualifications.
