Key Responsibilities\n

• Conduct comprehensive assessments of clients’ mental health needs, continuously adapting your therapeutic approach to meet their evolving requirements.

• Foster effective communication among various stakeholders, including supervisors, family members, and other care providers, to negotiate individual development plans and therapeutic goals.

• Maintain an organized calendar of appointments, home visits, and observations, utilizing data management practices to track and report on the progress of clients.

• Prepare for and actively participate in Interdisciplinary Team Meetings and training sessions, ensuring that the entire team is equipped to implement the program plan.

• Deliver follow-up support through additional interviews and observations, enabling proactive adjustments to treatment plans based on client feedback and observed outcomes.

• Document detailed progress notes on monthly and quarterly bases to ensure transparency and accountability within the treatment process.

• Engage in the community as a liaison between the public and mental health services, leveraging your expertise to support outreach and educational efforts.

Qualifications\n
