Key Responsibilities\n

• Adjust counseling techniques and strategies in response to clients' evolving needs, ensuring tailored care is delivered.

• Facilitate effective communication with a network of stakeholders including supervisors, team members, client families, and external partners to promote patient development.

• Maintain a well-organized calendar of appointments, home visits, and case notes, allowing for systematic tracking of client progress.

• Conduct home visits as necessary to obtain a comprehensive understanding of the clients' environments and to assess their needs effectively.

• Implement the established program strategy with fidelity, tracking outcomes and making modifications as necessary.

• Participate actively in interdisciplinary team meetings, sharing insights and collaboratively developing strategies for client care.

• Prepare detailed progress notes on a monthly and quarterly basis to accurately reflect client development and services rendered.

• Provide educational training sessions to new staff and team members, ensuring understanding and proficiency in program methods and documentation practices.

• Conduct follow-up interviews and observations to verify that ongoing client needs are met expediently.

• Act as a bridge between the public and mental health communities, contributing to community outreach initiatives that promote awareness and development.

• Work on continuous improvement of training materials aimed at enhancing program effectiveness and client outcomes.

Patient Population / Client Focus\n

Your client engagement will encompass a diverse population, including both adults and youth, who face a variety of mental health challenges such as anxiety, depression, trauma, and substance abuse issues. You will leverage your skills in assessment and diagnosis to craft individualized treatment plans, ensuring cultural competency and responsiveness to the unique experiences of each client.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• An active and unrestricted licensure as a Professional Counselor (LPC) within the state of New Jersey.
