While the majority of your work will be conducted remotely, you will also collaborate with an outpatient office team and occasionally conduct home visits to ensure the comprehensive needs of our clients are met.

Key Responsibilities\n

• Conduct regular assessments and engage in ongoing evaluation of clients’ changing needs.

• Actively facilitate open communication among supervisors, direct care staff, family members, and other stakeholders to promote holistic client development.

• Maintain an organized schedule of appointments, home visits, and observations to accurately track client progress and outcomes.

• Ensure effective implementation of treatment plans and regularly update them based on client feedback and changing circumstances.

• Participate in interdisciplinary team meetings to collaboratively discuss case management and strategize interventions tailored to individual client goals.

• Document all interactions meticulously, including recording monthly and quarterly progress notes to ensure compliance with legal and ethical standards.

• Organize and facilitate training sessions for staff to enhance their understanding and execution of treatment protocols and documentation requirements.

• Conduct follow-up interviews and home visits to ensure continuous support and adjust interventions as necessary.

• Serve as a liaison between the community and mental health resources, helping bridge gaps and facilitate access to care.
