Key Responsibilities\n

• Conduct comprehensive assessments of clients’ mental health needs to tailor individualized treatment plans that drive therapeutic progress.

• Maintain open and consistent communication with supervisors, treatment staff, family members, and other stakeholders to review and discuss client progress.

• Provide a structured schedule for appointments, ensuring all home visits, observations, and follow-ups are organized efficiently.

• Coordinate necessary home visits for in-depth assessments, including reviews of clients' records and interviews with family members.

• Oversee the implementation and effectiveness of treatment plans, adjusting strategies as necessary based on ongoing evaluations.

• Participate actively in interdisciplinary team meetings, contributing insights and collaborating with fellow professionals to enhance client outcomes.

• Document monthly and quarterly progress notes, capturing relevant developments and adjustments in client care.

• Organize and lead training sessions to ensure staff members are well-versed in the program plans and required documentation processes.

• Conduct follow-up interviews and home visits, actively engaging with clients to address their evolving needs.

• Serve as a liaison with the community and stakeholders in the mental health field to promote development initiatives.
