Key Responsibilities\n

• Conduct ongoing psychological assessments and evaluations to tailor therapeutic interventions that meet individual patient needs.

• Effectively communicate with multidisciplinary teams, including supervisors, direct care staff, family members, and other stakeholders to relay patients' progress and therapeutic needs.

• Create and maintain a tentative schedule for appointments and follow-up activities, ensuring timely monitoring of patient progress.

• Coordinate home visits and in-home observations, which involve reviewing patient charts and conducting interviews to gather comprehensive insights into patient well-being.

• Implement and monitor the effectiveness of individualized program plans, adjusting as necessary to improve patient outcomes.

• Participate in Interdisciplinary Team Meetings and contribute insights to enhance collaborative care.

• Document monthly and quarterly progress notes while adhering to legal and ethical standards of practice.

• Organize and deliver training sessions to certify staff on program implementation and documentation practices.

• Provide consistent follow-up care, including interviews and home visits, to ensure clients receive continued support that allows for adjustments in their treatment as needed.

• Maintain accurate and organized records, ensuring that information is easily accessible to authorized parties.

• Act as a liaison with the community and mental health organizations, aiding in the development of goals and services that benefit client populations.

• Assist in the creation of training materials aimed at improving program delivery and client satisfaction.

Patient Population / Client Focus\n

The LPC will focus on treating a diverse range of individuals, including both adults and youth. Qualifications\n
