Work Environment

You will work Monday to Friday, ensuring consistent access to care for our patients while collaborating with a passionate and skilled team of healthcare professionals.

Key Responsibilities

• Conduct thorough patient evaluations by reviewing medical history, performing specific assessments, and documenting results to devise appropriate treatment plans.
• Communicate findings and treatment plans clearly to patients and their families, ensuring that they are actively involved in goal-setting processes.
• Implement individualized treatment protocols based on evaluation outcomes and ongoing patient assessments, adapting to the evolving healthcare landscape.
• Collaborate with a multidisciplinary team of healthcare professionals to optimize patient care, recognizing changes in patient conditions and modifying treatment plans accordingly.
• Manage discharge planning and follow-up care, ensuring accurate and timely documentation of patient encounters, including baseline evaluations, progress notes, and discharge summaries.
• Maintain productivity standards while effectively managing time and schedules, ensuring timely attendance for patient sessions and team meetings.
• Participate in departmental initiatives to promote seamless patient access to care and contribute to the achievement of departmental goals and objectives.
• Engage in ongoing professional development by pursuing relevant continuing education opportunities and mentoring students as a clinical instructor.
• Strive for exceptional customer service, fostering strong relationships between patients, their families, and our healthcare team.

Patient Population / Client Focus

In this role, you will primarily work with individuals recovering from orthopedic conditions, including post-operative surgery, sports injuries, and chronic pain syndromes. Your expertise will be instrumental in helping patients regain mobility, strength, and confidence. You will engage with a diverse patient demographic, including active adults, seniors, and athletes, to tailor treatment plans that best suit their specific needs and rehabilitation goals.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program is required.
• Current state licensure as a Doctor of Physical Therapy by the Physical Therapy Examining Committee.
• A minimum of two (2) years of relevant clinical experience is essential.
• Certification in CPR is required to ensure the safety and well-being of our patients.

Preferred Experience

While not mandatory, experience in orthopedic rehabilitation and familiarity with evidence-based treatment approaches and patient management systems can further enhance your contributions to our team.
