Work Environment

This full-time position is onsite, allowing therapists to engage directly with patients in the comfort of their homes. Our home care facility provides a supportive atmosphere where you will work closely with various healthcare professionals, including physicians, nurses, and occupational therapists, to create cohesive treatment plans that ensure optimal patient outcomes.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, identifying impairments, functional limitations, and disabilities that may hinder their daily activities. This may include assessing joint mobility and muscle strength based on individualized patient needs.

• Develop and implement personalized treatment plans in collaboration with physicians' directives, adapting strategies to suit the patient's age, development stage, and specific therapeutic goals.

• Thoroughly document all patient care activities, including evaluations, treatment plans, progress notes, and discharge summaries, ensuring clarity and professionalism in communication.

• Supervise the performance of Physical Therapy Assistants (PTAs) and support personnel, guiding their contributions to patient care and fostering a collaborative environment that promotes therapeutic results.

• Regularly assess the effectiveness of therapeutic interventions and adjust care plans as needed, utilizing outcome measures to report on progress to both patients and the healthcare team.

• Participate in interdisciplinary team meetings to discuss patient progress and adjust treatment plans, ensuring an integrated approach to patient care.

• Engage in patient education, empowering individuals with knowledge about their conditions, recovery process, and long-term health maintenance strategies.

The ideal candidate will work with a varied patient population, including individuals recovering from surgeries, managing chronic illnesses or disabilities, and those requiring supportive rehabilitation following major life changes. Qualifications

• Possession of a Bachelor of Science degree in Physical Therapy from an accredited institution.

• Current licensure as a Physical Therapist in the state of Virginia.

• Strong verbal and written communication skills required for effective documentation and patient interactions.

• Proficiency in computer and word processing applications to maintain accurate records and documentation.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy setting is highly preferred. Experience in home care is a plus, as it prepares you for the specialized demands of delivering therapy in patients' homes.

Compensation and Benefits
