• Conduct comprehensive assessments to evaluate patients' mental health needs, formulate individualized treatment plans, and conduct follow-up evaluations to track progress.

• Deliver short-term individual counseling and crisis intervention for patients experiencing acute mental health challenges, employing tailored therapeutic approaches to address their specific needs.

• Facilitate engaging therapy groups and workshops focused on personal development, coping strategies, and mental health awareness, bringing together patients to share experiences and learn from one another.

• Collaborate with healthcare teams across departments, ensuring integrated care by coordinating efforts with medical and support staff to optimize patient treatment and outcomes.

• Participate in outreach initiatives that may include organizing mental health awareness events, community workshops, and preventive care campaigns to educate the public about mental health issues.

• Adhere to ethical and legal standards by maintaining accurate and confidential records for all patient interactions, following established protocols for documentation and reporting.

The primary focus of this role is on patients within a hospice setting, often facing complex emotional and psychological challenges associated with end-of-life care, grief, and loss. Your understanding of the unique mental health issues affecting this demographic will be vital in crafting tailored interventions that support their emotional and psychological well-being.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).

• A Bachelor's degree in Social Work, Psychology, Counseling, or a related field; a Master's or Doctoral degree is strongly preferred.

• A deep understanding of the mental health challenges faced by patients in hospice settings, along with demonstrated proficiency in providing compassionate care.

• Exceptional communication and interpersonal skills to foster rapport with patients, families, and multidisciplinary teams.

• Familiarity with evidence-based practices and brief intervention strategies suited for high-need clinical environments.

• The ability to manage a diverse caseload, demonstrating flexibility to prioritize tasks in a fast-paced, often emotionally charged setting.

• Previous experience working in hospice, palliative care, or similar health settings is highly desirable.

• Proficiency in electronic health record (EHR) systems and telehealth platforms for effective patient management and documentation.

• Experience facilitating group therapy sessions or workshops that promote mental health awareness and skills development.

• Annual salary ranging from $80,000 to $100,000, commensurate with experience and qualifications.
