Job Description

Schedule:

Full-time, Monday through Friday
On-call rotation likely
Structured daily visit expectations

Key Responsibilities:

Deliver skilled nursing care directly to patients in their homes
Complete comprehensive assessments and OASIS documentation accurately and promptly
Coordinate with physicians and multidisciplinary teams to tailor individualized care plans
Manage wound care, medication administration, patient education, and monitor disease progression
Communicate any patient condition changes and ensure timely updates to the clinical team

Qualifications & Skills:

Active Registered Nurse (RN) license
At least 1 year of recent RN experience in home health
Proficient with OASIS documentation (required)
Familiarity with HomeCare HomeBase (strongly preferred)
Reliable transportation and valid drivers license
Confidence in managing a structured schedule and daily patient visits
