Key Responsibilities\n

As a Licensed Clinical Social Worker in our detox center, you will:

• Conduct thorough psychosocial assessments through patient interviews, observations, and reviews of medical records to identify emotional, social, and environmental factors influencing recovery.

• Provide individual and group counseling to patients and their families, focusing on emotional well-being and adjustment issues related to addiction and recovery.

• Collaborate with a multidisciplinary team of rehabilitation professionals to develop and implement personalized treatment plans that address the unique psychosocial needs of each patient.

• Assist patients in navigating available community resources, such as support groups, vocational rehabilitation programs, and housing assistance, to facilitate seamless reintegration.

• Engage in discharge planning, ensuring coordinated transitions for patients moving from the detox facility back to their homes or other care settings.

• Educate patients and families on community resources and post-rehabilitation services to provide continued support and promote long-term recovery.

Qualifications\n

To qualify for this position, candidates must possess:
