Key Responsibilities\n

As a Licensed Professional Counselor at Frontier Behavioral & Mental Health Clinic, you will be responsible for:

• Conducting patient intakes and assessments, focusing on diagnostic and functional evaluations, followed by recommendations for treatment plans in collaboration with primary care providers.

• Providing brief, focused interventions for patients requiring mental health services, ensuring to tailor approaches to individual needs.

• Monitoring patient progress through regular follow-up appointments and adjusting treatment plans based on their evolving needs.

• Working as an integral part of the primary care team to identify mental health and psychosocial issues, making referrals to specialty mental health facilities as necessary.

• Developing relapse prevention plans and helping patients maintain stable functioning in their daily lives.

• Documenting all patient interactions accurately and maintaining comprehensive treatment charts, ensuring that all progress is communicated effectively to the primary care team.

• Advising on self-management techniques and providing educational resources that empower patients to take an active role in their mental health recovery.

• Participating in clinic meetings and contributing insights about behavioral health services and areas for improvement or evaluation.

Qualifications\n

The ideal candidate will have:

Compensation and Benefits\n
