Intake Coordinator – Home Healthcare

At Peace Healthcare Agency is seeking an experienced Intake Coordinator to join our growing healthcare team.

Spanish-Speaking REQUIRED
Pay: $18–$25 per hour

We are looking for a highly organized, compassionate, and dependable professional with experience in healthcare intake, home care, admissions, care coordination, or a related healthcare role.

Key Responsibilities

• Serve as a primary contact for new clients, families, and referral sources

• Conduct intake calls and gather client information

• Explain services and guide clients through the intake process

• Coordinate assessments, appointments, and admissions

• Collect and track required documentation

• Follow up on pending referrals and information

• Maintain accurate client records

• Coordinate with internal teams for a smooth transition to services

• Handle calls, emails, and SMS professionally

• Address client and family questions and concerns

• Maintain confidentiality and follow HIPAA requirements

Qualifications

• Previous intake or healthcare coordination experience REQUIRED

• Fluent in English REQUIRED

• Spanish-speaking REQUIRED

• Excellent communication and organizational skills

• Strong follow-up and attention to detail

• Ability to multitask in a fast-paced environment

• Strong computer and documentation skills

• Professional, dependable, and compassionate

Experience in home care, healthcare, behavioral health, or disability services is a plus.
