This role is 100% remote, allowing for flexibility and the ability to work from anywhere while supporting our community health clinic's mission. Key Responsibilities\n

• Conduct thorough evaluations and assessments of children to determine needs and develop tailored intervention strategies.
\n
• Play a pivotal role in the Individualized Family Service Plan (IFSP) development and implementation, collaborating with families and multidisciplinary teams to outline goals and services.
\n
• Monitor and evaluate the effectiveness of intervention strategies, making necessary adjustments based on ongoing assessments and family feedback.
\n
• Provide expert support to families, helping them understand and utilize resources effectively to enhance their child's development.
\n
• Adhere to all federal and state regulations governing social work practices and ensure compliance with organizational standards.
\n
• Establish rapport with children and families, fostering a trusting environment conducive to open communication and engagement.
\n
• Utilize evidence-based practices and natural learning environment techniques to guide families in enhancing the developmental outcomes for their children.
\n
• Regularly coach families through challenges, offering techniques and information to support the child’s growth.
\n
• Be an active, proactive participant in team meetings, sharing insights and feedback regarding client progress and departmental goals.

Patient Population / Client Focus\n

Your primary focus will be working with children, particularly infants and toddlers, who may have disabilities or developmental delays. You will assist these young individuals in developing essential physical, cognitive, social, emotional, and communication skills critical in the early stages of life. By strategically implementing personalized interventions, you will play a vital role in shaping positive developmental trajectories.

Qualifications\n

• A Master's Degree or Doctorate in Social Work.

• Active licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.

• A valid driver’s license is required.
\n
• A minimum of one year of experience working with early intervention services, ideally with young children.

• Proficient in various learning styles to tailor interventions appropriately for each child.
