Work Environment

This position is carried out onsite in the homes of our clients, allowing for a personalized approach to therapy, which is pivotal in home healthcare settings. Key Responsibilities

The Physical Therapist will direct physical therapy treatment with the goal of preventing disability, relieving pain, and enhancing muscle function. Key responsibilities include:

• Conducting assessments of clients' physical and functional capabilities, utilizing diagnostic tests to develop tailored treatment plans.
• Supervising Physical Therapy Assistants in implementing care plans while ensuring compliance with state regulations.
• Administering therapeutic modalities, including manual therapy and exercises, while educating clients and families on activities that promote independence.
• Evaluating home environments and making recommendations to optimize client rehabilitation and safety.
• Documenting all clinical records clearly, including evaluations, care plans, and discharge summaries, in accordance with legal and organizational standards.
• Participating actively in interdisciplinary team meetings to discuss client progress and adapt treatment plans as necessary.
• Providing guidance to clients and families about therapy devices and the overall physical therapy program.
• Maintaining compliance with regulatory standards and submitting necessary reports and documentation promptly.

Patient Population / Client Focus

This role primarily serves adults and elderly individuals who require therapeutic interventions to manage mobility issues, recover from illness, or rehabilitate from surgery. The therapist must be adept at addressing diverse health conditions, ranging from post-operative recovery to chronic pain management. Qualifications

The ideal candidate for the Physical Therapist role will possess the following qualifications:

• A Doctorate degree in Physical Therapy (DPT) and a valid state license in good standing.
