Key Responsibilities

• Conduct thorough assessments of clients' psychological, emotional, and social needs to inform individualized treatment plans.
• Implement evidence-based therapeutic interventions aimed at enhancing client well-being and coping mechanisms.
• Maintain meticulous and confidential clinical records to ensure compliance with regulatory and ethical standards.
• Collaborate proactively with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to provide holistic care.
• Educate and support clients and their families, offering guidance and resources tailored to their unique circumstances.
• Continuously seek professional development by staying informed about the latest research and developments in mental health.

Patient Population / Client Focus

You will primarily work with patients within a hospice care setting, addressing emotional and mental health concerns related to chronic illness, terminal diagnoses, loss, and grief. Our clients often require a high level of sensitivity, and your role will be to provide therapeutic support that enhances their quality of life and emotional well-being during this critical time.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in New York state is mandatory.
• At least 2 years of clinical experience in mental health, preferably in a hospice or palliative care environment.
• Proficiency in solution-based psychotherapy interventions and trauma-informed care approaches.
• Strong communication and interpersonal skills necessary for building rapport with clients and families.
• Excellent analytical, problem-solving, and critical thinking abilities.

Preferred Experience

• Previous experience working in hospice care or with clients facing end-of-life issues is highly desirable.

• Competitive salary range of $115,000 to $130,000 annually, commensurate with experience.
