Key Responsibilities

• Conduct thorough clinical assessments, utilizing standardized assessment tools and interviews to develop tailored treatment plans that address the unique needs of hospice patients.
• Provide evidence-based therapeutic interventions, employing techniques such as cognitive-behavioral therapy (CBT) and solution-focused therapy to assist patients and their families in coping with the emotional aspects of terminal illness.
• Maintain precise and timely clinical records in compliance with state regulations and organizational policies, ensuring all documentation is complete and up-to-date.
• Work collaboratively with an interdisciplinary team, including physicians, nurses, and other social workers, to coordinate comprehensive care plans and address complex patient needs.
• Offer educational resources and emotional support to both patients and families, guiding them through difficult decisions and facilitating discussions about end-of-life care and grief.
• Engage in ongoing professional development by staying current with research and advancements in social work and mental health practices within the context of hospice care.

Patient Population / Client Focus

In your role as an LCSW, you will be working predominantly with terminally ill patients and their families. Qualifications

• A Master’s degree in Social Work from an accredited institution.
