Work Environment

As an LCSW with Life and Health Mental Solutions, you will operate in a remote setting, which allows you to engage with clients through virtual platforms while maintaining flexibility in your work schedule. This role is essential in a hospice center context, where you will navigate the emotional complexities associated with end-of-life care. You will collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare providers, to support the mental health needs of clients and their families.

Key Responsibilities

• Conduct comprehensive mental health assessments tailored to the hospice setting, evaluating clients’ emotional states and any presenting concerns, including grief, anxiety, and depression.
• Develop individualized treatment plans in partnership with clients and families, focusing on enhancing emotional well-being and facilitating coping strategies during difficult transitions.
• Provide evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and mindfulness-based approaches, addressing the unique mental health challenges present in hospice care.
• Facilitate individual counseling sessions and, if appropriate, support group sessions for family members, helping them navigate their feelings of loss and grief.
• Collaborate actively with healthcare team members to coordinate care, ensuring that mental health support is integrated into the overall treatment plan for clients.
• Offer crisis intervention services, employing de-escalation techniques and rapid assessment to respond to urgent mental health needs that may arise.
• Educate clients and families about mental health resources, treatment options, and community support, empowering them to engage with available services.
• Maintain meticulous records of client interactions, treatment plans, and progress notes in accordance with professional, legal, and ethical standards.
• Participate in biweekly case consultations and team meetings to discuss client progress, share insights, and foster a culture of continuous improvement.
• Stay informed of emerging practices in mental health care as they relate to hospice settings and adjust treatment modalities accordingly.

Patient Population / Client Focus

The primary client population served in this role includes individuals in hospice care and their families, many of whom are experiencing significant emotional distress as they confront terminal illness and potential loss. You will work closely with diverse backgrounds, navigating complex emotional landscapes while providing support that honors each client's unique situation.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Current and valid LCSW licensure in the state of Illinois.
• Minimum of two years of post-licensure experience, with a preference for those with a background in hospice, palliative care, or similar settings.
• A deep understanding of mental health disorders, particularly as they relate to grief and loss, and familiarity with various therapeutic approaches.
• Proficiency in conducting detailed mental health evaluations and developing appropriate treatment plans based on individual client needs.
• Strong crisis intervention capabilities, enabling composure in high-caliber emotional situations.
• Exceptional communication and interpersonal skills, crucial for building rapport with clients and their families during vulnerable moments.
• Experience with electronic health record (EHR) systems is preferred.

Preferred Experience

• Experience providing mental health support in a healthcare or educational setting.
• Knowledge of trauma-informed care methodologies and practices.
• Prior involvement in interdisciplinary teams, showcasing your ability to collaborate with healthcare providers.

Compensation and Benefits

The salary range for this position is competitive, with annual compensation between USD 80,000 and 100,000, commensurate with experience and qualifications.
