Key Responsibilities\n

As the LCSW, you will report directly to the Social Work Manager and will be responsible for:

• Conducting bio-psychosocial evaluations to identify and eliminate barriers to treatment, ensuring optimal patient health statuses.

• Developing detailed treatment plans tailored to individual needs, utilizing both individual and group therapy modalities.
\n
• Providing counseling and support to patients and families, helping them navigate the complexities of hospice care while promoting understanding and adherence to treatment recommendations.

• Collaborating with a multidisciplinary team, including medical and support staff, to create comprehensive care plans and promote a cohesive treatment approach.

• Documenting all social work activities in compliance with departmental standards and maintaining accurate medical records.

Patient Population / Client Focus\n

This role primarily focuses on patients receiving hospice care, including individuals facing chronic illnesses, terminal conditions, and significant end-of-life decisions. To be considered for this position, candidates must possess:

• A Master’s Degree in Social Work from an accredited institution and a current license as a Licensed Clinical Social Worker (LCSW) by the State Board of Social Work Licensure.

Compensation and Benefits\n

This position offers a competitive salary range of USD 85,000 – 100,000 annually.
