Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families to identify strengths and challenges related to the hospice journey.

• Develop individualized, patient-centered care plans that address both physical and emotional needs, utilizing various therapeutic modalities.

• Implement evidence-based interventions, such as solution-focused therapy and motivational interviewing, to empower patients and enhance their quality of life.

• Regularly monitor and document patient progress, adjusting treatment plans as necessary to meet established goals and enhance emotional well-being.

• Collaborate with interdisciplinary teams, including nurses, physicians, and palliative care specialists, to ensure a cohesive approach to patient and family support.
