Work Environment\n

This is a fully remote position, allowing you the flexibility to work from home while engaging with clients through virtual consultations and telehealth services. Your schedule will predominantly align with a full-time commitment, operating Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours depending on client availability. This role requires adaptability, as you may be required to participate in on-call rotations to address urgent client needs outside of regular business hours.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of senior clients, taking into account emotional, mental, and social factors affecting their well-being.

• Develop personalized care plans in collaboration with clients and their families, ensuring that each plan aligns with the individual's goals and preferences.

• Provide counseling and emotional support to seniors, facilitating their coping strategies during significant life transitions, such as health changes, loss of independence, or grief.

• Advocate for clients' rights and help them access community resources, ensuring they have the support systems necessary for their health and well-being.

• Collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to create a seamless care experience for clients.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
