Work Environment\n

This full-time role allows for flexible scheduling, enabling you to work remotely and adapt your hours to meet the needs of patients and their families. Responsibilities may include day, evening, and weekend shifts, particularly during peak periods or as necessitated by the needs of the hospice services. Successful candidates may also be expected to participate in on-call rotations to ensure continuous support and accessibility for patients.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients’ emotional, social, and financial needs.

• Provide individual and group counseling focused on topics such as grief, adjustment to illness, and effective coping strategies.

• Develop and implement comprehensive discharge plans, working collaboratively with medical teams to ensure the transition to post-discharge services.

• Coordinate with local community organizations to facilitate appropriate follow-up care, ensuring continuity and quality of care.

• Offer emotional support and education to patients and families, empowering them to make informed treatment decisions.

• Provide crisis intervention and counseling in challenging situations, maintaining a calm and supportive demeanor.

• Participate in hospital committees and contribute social work expertise to multidisciplinary discussions to enhance patient outcomes.

• Maintain comprehensive documentation in compliance with all hospital policies and legal standards.

Patient Population / Client Focus\n

The LCSW will work predominantly with hospice patients and their families, providing tailored support during one of the most critical phases of life. Qualifications\n

Compensation and Benefits\n
