Work Environment\n

This position is based remotely, allowing you to provide services to seniors and families from the comfort of your designated workspace. While you will primarily conduct assessments and meetings virtually or over the phone, occasional in-person visits to our detox center may be necessary to support clients directly. The work schedule is Monday to Friday from 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend hours based on client needs. On-call responsibilities may also be required to ensure timely support for our clients.

Key Responsibilities\n

• Conduct detailed psychosocial assessments to identify the emotional, mental, and social factors affecting seniors’ well-being.

• Develop and implement personalized care plans in collaboration with clients, their families, and interdisciplinary team members.

• Provide counseling and emotional support that addresses the specific challenges seniors face during life transitions, such as health changes or loss of independence.

• Actively advocate for client rights and facilitate access to essential community resources to ensure comprehensive support.
\n
• Regularly monitor the well-being of clients through virtual check-ins, addressing any concerns regarding their living conditions or safety.
\n
• Serve as a mediator to resolve conflicts among family members involved in the client's care, fostering better communication and understanding.
\n
• Educate seniors and their families about available resources, including support groups and services that can improve their social engagement.
\n
• Ensure accurate and timely documentation of client interactions, assessments, and progress notes in our electronic health record (EHR) system.

• Stay updated on current social work practices, applicable policies, and ethical standards, ensuring full compliance in all matters.

Patient Population / Client Focus\n

This role primarily focuses on senior clients, including those with complex psychosocial needs such as mental health challenges, substance use issues, and those transitioning from various living situations or care facilities. Your work will significantly impact seniors facing critical life changes, helping them access the right resources and support to navigate these challenges with dignity and respect.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Master Social Worker (LMSW).

• Minimum of two years’ experience in a social work role, preferably within a community health, geriatric, or similar care setting.

• Comprehensive knowledge of aging-related issues, social work theories, and community resources.
\n
• Strong assessment, counseling, and individualized care planning skills.
\n
• Ability to empathize with clients and establish meaningful rapport with seniors and their families.

• Excellent communication and interpersonal skills to effectively collaborate with diverse teams.

• Proficient in electronic health record (EHR) systems and Microsoft Office applications.

Preferred Experience\n

• Previous experience working with seniors or in a geriatric setting is highly desirable.

• Knowledge of addiction recovery processes or experience working in detox environments is a plus.

• Demonstrated ability to address multi-faceted client situations with a holistic approach.

Compensation and Benefits\n

The salary for this position ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications.
