Key Responsibilities\n

• Liaison with Families and Providers: Serve as a critical connection point between clients, their families, and outpatient service providers to ensure seamless communication and comprehensive care.

• Insurance Collaboration: Engage with insurance companies to facilitate pre-certification and manage utilization effectively, ensuring clients receive the necessary support and resources.

• Aftercare and Discharge Planning: Actively participate in developing discharge plans and aftercare strategies tailored to each client’s unique circumstances, ensuring ongoing support post-hospitalization.

• Therapeutic Meal Support: Provide therapeutic meal support and guidance, modeling healthy relationships with food to clients, particularly those experiencing end-of-life challenges.

• Client-Centered Care: Deliver safe, empathetic, and affirming care to clients, considering their intersecting identities and needs, to foster a sense of trust and safety.

Patient Population / Client Focus\n

This role specifically focuses on providing care to older adults, particularly those in hospice settings, navigating the complexities of terminal illness. Understanding the unique challenges faced by this demographic will allow you to tailor your approach and interventions accordingly, promoting dignity and comfort in their final stages of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A valid LCSW license from the state of Colorado is mandatory.

Compensation and Benefits\n
