Key Responsibilities\n

• Develop personalized treatment plans based on comprehensive assessments for individuals, couples, families, and group therapy sessions.

• Conduct regular evaluations to monitor client progress, adjusting treatment approaches as necessary, and providing referrals to other services when warranted.

• Offer timely crisis intervention services to clients experiencing acute distress, ensuring their safety and wellbeing.

• Foster strong, therapeutic relationships with clients and collaborate with family members and community resources to enhance holistic support.

• Maintain accurate, comprehensive documentation of interactions and services provided to ensure compliance with organizational standards.

• Participate in team meetings, quality assurance activities, and continuous in-service training to improve service delivery and clinical performance.

Patient Population / Client Focus\n

Your primary focus will be on providing services to clients in a hospice setting, which involves working with individuals and families navigating end-of-life care. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid Licensed Clinical Social Worker (LCSW) designation in the state of Colorado.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 70,000 to 95,000 per year, commensurate with experience and qualifications.
