• Conduct comprehensive assessments of seniors' psychosocial needs through interviews, surveys, and direct observation to develop personalized care strategies.

• Design and implement tailored care plans that align with clients’ specific goals, preferences, and available resources, ensuring active family involvement throughout the process.

• Provide individual and family counseling, helping seniors and their loved ones navigate emotional challenges, transitions, and conflicts.

• Act as an advocate for clients, facilitating access to community services, educational resources, and support groups that enhance senior living.

• Regularly monitor client well-being through scheduled virtual visits, adjusting care plans as needed based on changes in circumstances or health status.

• Maintaining accurate and comprehensive documentation of all client interactions, including assessments, care plans, and progress notes, to ensure continuity of care.

• Participate in team meetings and case conferences to collaboratively address client issues and develop multi-faceted care approaches.

• Educate clients and their families about available community resources, fostering increased social engagement and support.

• Stay informed about best practices in social work, aging issues, state regulations, and ethical standards to uphold the quality of care.

This role primarily serves elderly clients and their families, many of whom face complex medical, psychological, and social challenges. The licensed social worker will work closely with clients experiencing aging-related issues, including chronic illnesses, mental health concerns, and the impact of isolation and loss.

• A Master's degree in Social Work (MSW) from an accredited program.

• A current and valid state licensure as a Licensed Master Social Worker (LMSW) in Florida.

• A minimum of two years of relevant experience in a social work capacity, particularly with geriatric populations or within community health settings.

• Strong understanding of social work principles, aging processes, and available community resources for seniors.

• Exemplary counseling, advocacy, and assessment skills, coupled with the ability to build trusting relationships with clients.
