Key Responsibilities\n

• Actively engage in discharge planning processes, ensuring a smooth transition for patients back to their homes or to other appropriate care settings, tailored to meet their unique needs.

Compensation and Benefits\n

The annual salary for this position ranges from USD 75,000 to 100,000, commensurate with experience and qualifications.
